Admission Documents and Instructions
ShortStop is not an emergency shelter. The application process starts with filling out the following forms and faxing them to the program at (617) 628-3915. Download any of the PDF documents below and follow the instructions included on this page.- The Intake Assessment and Referral Form needs to be filled out by the perspective resident with as much detail and honesty as possible.
- The Verification of Homelessness Form needs to be filled out by a case worker or agency representative working with the youth who can verify that they are homeless.
- The References Form must include 3 references. These can be anyone who knows the youth-case managers, family members, friends, or teachers are all able to complete this form.
Once all materials have been received by ShortStop, the potential resident will be contacted for an interview. Initial interviews are done as soon as possible after receiving the application materials. If you feel you should have been contacted for an interview and have not been, please feel free to give us a call and check on your application. If applicants are a good match for the program, they will be asked back for a second interview once a bed becomes available.
